With the creation of the APSF, Ellison Pierce, Jr., M.D., set forth a challenging vision – “No patient shall be harmed by anesthesia.” A decade later in his Rovenstine Lecture, he celebrated the robust progress in patient safety while reminding us to remain steadfast in the elimination of harm. This 2018 Ellison C. Pierce Patient Safety Memorial Lecture will sharpen Dr. Pierce’s vision by looking at the relationship between zero defects and source inspection. The lecture will consider novel ways that all members of our community can work together to make Dr. Pierce’s vision a reality.
