Perioperative Surgical Home: The Future Is Now

In his letter to the editor, Dr. Zornow raises a number of questions regarding the Perioperative Surgical Home (PSH). What is it, how does it relate to anesthesiology, do we have the expertise to provide pre- and postoperative care, and who will pay for it?
In this Monitor issue, along with previous issues, a number of articles detail the PSH, its definition and some implementations. We shall not repeat them.
The larger question is this: how will the profession of anesthesiology continue to survive and thrive in a time when medicine is moving toward value-based methodology? Traditional intraoperative care alone will not be sufficient to demonstrate the value of physician anesthesiology when the intraoperative period, plus a short period of time before and after, is only a tiny fraction of the entire episode of care. We ignore this problem at our own risk.
